
Lakes Homes & Program Development, Inc., provides the following services:
1. Intermediate Care Facility for the Mentally Retarded (ICF/MR)
This facility was newly constructed in 1980 as an ICF/MR Class B facility with a license capacity of nine (9). It is licensed by the Minnesota Department of Human Services to serve adults with developmental disabilities and by the Minnesota Department of Health as an Intermediate Care Facility and Supervised Living Services (SLS). The primary source of funding is the Minnesota Department of Human Services Medial Assistance with secondary sources of funding from Housing & Urban Development under the Section Eight (8) Subsidy Program and private pay.
2. Corporate Adult Foster Care (AFC) with Supportive Living Services (SLS)
These programs are licensed as Corporate Adult Foster Care by the Minnesota Department of Human Services in the county where they are located for not greater than four (4) individuals per home. Lakes Homes currently operates AFC programs in Becker, Otter Tail and Mahnomen counties. Each program is licensed as Supportive Living Services Waivered through the Minnesota Department of Human Services to serve adults with developmental disabilities.
3. Corporate Adult Foster Care (AFC) for Persistent and Chronic Mental Health Services
This program, outside of Detroit Lakes, serves four (4) adults with Persistent and Chronic Mental Health needs. The primary source of funding for this program is through Becker County Human Services Mental Health program grant with secondary funding source of Adult Foster Care and private pay.
